Consent for Medical Treatment

In the case of a medical emergency, I understand that every reasonable attempt will be made to contact the Parent(s)/Guardian(s) or another designated emergency contact. However, in the event that I cannot be reached, I give my permission to the Educators of Wonderforest Preschool, LLC to help my Child and secure emergency medical treatment. (Local hospital, where educators intend to go, as long as it is available, is New York Presbyterian Brooklyn Methodist, 506 6th Street, Brooklyn, 11215.) I agree to pay for any charges for emergency medical treatment that are not covered by my personal health insurance. This acknowledgement and consent applies for the duration in which my child attends class.
Acknowledgement of Risk

I acknowledge that there are risks inherent in any youth activity, including – but not limited to – injury arising from participation in outdoor physical activity. I acknowledge that all risks cannot be prevented, and assume those beyond the reasonable control of the teachers and staff. In consideration of being permitted to participate in classes at Wonderforest Preschool, on behalf of myself, my family, my heirs, and my assigns, I hereby release and hold harmless Wonderforest Preschool, its teachers and its staff from any liability for injury, loss, or death to the Child. In order to minimize risks to my Child and others, I will take responsibility to make sure that my Child is prepared for all activities, dressed appropriately for the weather, and is in good health for each class. I am also aware that Prospect Park, Pacific Park, St. Marks Community Garden, Dean Street Field are public parks with rough terrain and wild animals, and open to the general public for various activities. I appreciate and accept that risk and waive any right to pursue legal remedies associated with inherent risks of the parks and other outdoor locales.
COVID-19 Acknowledgement of Risk

Parents may have reservations about having their child participate in an in-person program due to the potential risks associated with COVID-19. Although Wonderforest Preschool will comply with the guidance provided by the New York State Department of Health, Wonderforest Preschool cannot eliminate the risk of exposure or spread of COVID-19 during the in-person program. By allowing your child to participate in the in-person program operated by Wonderforest Preschool, you understand and accept these potential risks, and agree to abide by the New York Department of Health Guidelines.
Park Trips & Local Outings

Each class at Wonderforest will aim to visit Prospect Park a minimum of one time each week. Safety is our primary concern, and severe weather advisories/alerts, like heat advisories, thunderstorms, and air quality alerts, as well as staffing limitations, may result in postponement or cancellation of park trips.
On days when we do not visit Prospect Park, classes will have the option to take neighborhood walks to Pacific Park, St. Marks Community Garden, or Dean Street Field, with the option to stay for snack or lunch. A trip schedule for each individual class will be shared ahead of time.
I have read, understand, and agree to the terms of “Park Trips & Local Outings” and give permission for Wonderforest to take my child on scheduled trips outside of the school building.
Sunscreen & Bugspray Acknowledgement

Application of sunscreen and bugspray is required during class. Wonderforest will provide these items for all children as needed, or if preferred, families may send in a specific kind for their child to use. Families should send additional sun protecting clothing to be worn as needed throughout the day. Families acknowledge that teachers will assist their children in applying sunscreen and bug spray as needed.
Food Preparation & Safety

Families are responsible for sending children with non-perishable snacks and lunches each day. Wonderforest is unable to provide refrigeration or reheating of food. Families must ensure that all non-perishable food is packed in insulated containers with ice packs to maintain safe food temperatures. If Wonderforest deems a child’s snack or food has not been safely packed, families will be responsible for replacing the food ASAP.
Indoor/Outdoor Toileting Affirmation and Release

Absences & Late Dropoff

Families will notify teachers via Brightwheel of any absences or lateness as soon as possible each day. In the rare event that a child must arrive late to class, dropoff can occur no later than 11 am. Families may need to meet their child’s class in an outdoor location if they are running late that day.

Schedule, Tuition, and Payment

Tuition is non refundable, non transferable and due in full prior to your child attending class.
All payments are non-refundable, non-transferable, and must be submitted by date noted on your invoice to secure your child’s spot in our classes. By enrolling in our classes, families assume responsibility for the full balance due. Families are responsible for the full balance due, regardless of the days their child actually attends. No refunds or credits given for classes missed, including those due to travel, illness, schedule conflicts, or other circumstances beyond our control, including weather, city related closures or closures related to utilities.
